What is our new patient procedure?
In order to get started, you will need to contact our office to schedule an evaluation. You will then attend an “Initial Evaluation” during which your child’s PT will review your child’s medical history with you, conduct a physical and functional assessment of movement and, if needed, complete a standardized test to assess your child’s developmental performance compared against peers. You will then help the PT establish goals and a plan of care for your kiddo that work well for your family.

Please be as thorough as possible.
You will bring your prescription, insurance information and documentation to your first appointment. You can also fax or email the paperwork to our office prior to your appointment.

Do I need a doctor’s referral?
A prescription from your child’s pediatrician or specialist for “physical therapy evaluation & treatment” is required for any PT interventions.
